Please use the form below to contact High Peak First Aid Training. Please ensure you read all details below before completing the online application. By completing the booking form we assume you have read and will comply with the terms and conditions which can be found at the link on the bottom of the page.

On receipt of this form we will notify you of your course date and reserve a place for you for up to 7 days.
- A place can only be guaranteed on receipt of the full amount by bank transfer, unless by written arrangement. For late bookers cash can be paid on the first morning of the course. Receipts are issud for all payments.
- On receipt of payment, course joining instructions will be sent by email, so please fill out the email section on the form below.
- Please refer to accommodation if necessary.
- It is essential that we are able to contact you by email and have at least one telephone number to ensure that all details reach you.
- Teachers, please will you ensure we have a mobile no and a preferably a personal email as due to the nature of your job you can be difficult to contact, thank you.
For bookings at your venue:
- On receipt of this form we will contact you by telephone to ensure your requirements are met.
